SCOPE AND PURPOSE OF POSITION:
The Care Manager provides comprehensive care management services to children and adults presumed to meet eligibility requirements of New York State Health Home program. They will gather documentation that confirms eligibility for the program and work with clients to assess and identify areas requiring supportive services. (Children’s Health Homes) The care manager works with children, families and service providers to complete the Child & Adolescent Needs and Strengths Assessment, which will guide them in creating a comprehensive Plan of Care. The care manager will collaborate with client’s service providers and with client/family to help achieve identified goals on Plan of Care. (Adult Health Homes) The care manager works with Adults in creating a comprehensive Plan of Care. The care manager collaborates with the client’s other providers to help achieve identified goals on Plan of Care. Care manager will assess the need for additional services, such as Home and Community Based Services for both children and adults enrolled in the Health Homes program. The Care Manager will be required to document all contact related to client’s case in an electronic health record and monitors the need for additional interventions based on client need.

Position Overview:
The Salvation Army Reading Corps is seeking a caseworker to provide case management and emergency crisis assistance to families in need. Caseworkers are trained in programs designated to help individuals and families overcome barriers leading to self-sufficiency, such as utility disconnects, rental assistance, utility customer assistance programs, prescriptions, photo ID’s, bus passes, and home heating assistance. Maintain small responsibilities within The Salvation Army’s Food Pantry Program.
Under the direction of the Facility Manager, the Porter helps to ensure the facility is properly cleaned, safe and sanitary for all residents, staff and visitors into the building. The Porter helps to complete work orders, minor building repairs and completes the cleaning of all vacant units within the regulatory time frame of four hours.

Hours Per Week: 37.5
Schedule: Mon-Fri; 8:00 am – 4:00 pm (includes two 15-minute paid breaks and one 30-minute unpaid meal break per day)
Scope & Purpose of Position: Maintain the safety and cleanliness of the GHAS building and surrounding property. Provide equipment and furniture arrangements for meetings and other activities. Troubleshoot and handle light maintenance issues at The Right Place (TRP), the Marshall House Family Shelter, and the North End Corps. Complete driving duties as requested by supervisor. Work with the supervisor to request contractors.
