SCOPE AND PURPOSE OF POSITION:
Care Manager provides outreach and enrollment services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program. They gather assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work with children, families and service providers to create a comprehensive Plan of Health Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. Care Manager documents progress and arrange for eventual transition between health homes as necessary, or from health homes to other levels of service following changes in eligibility.
Classification: Full-Time, Exempt, 40 hours per week, Monday-Friday, 8:30 a.m. – 5:00 p.m.
Salary: $56,000-$59,000
Job Focus: Manages the housing/homeless programs for Central Ohio and provides supervision for housing employees. Serves as the grants administrator for all CAS housing programs. Writes program grants, monitors grant requirements and financial policies, and provides reports to funders for the Franklin County and Regional Housing programs.

SCOPE AND PURPOSE OF POSITION:
The Case Manager position exists to assist in providing services for the Domestic Violence/Rape Crisis Program of The Salvation Army Anew Center in Chautauqua County. Based at The Salvation Army's South or North County Office, the Case Manager will be responsible for the implementation of domestic violence, sexual assault, and rape crisis services throughout Chautauqua County. The Case Manager Position also assists with housing of homeless women as needed.
Location: Camp CONNRI (Ashford, CT)
Status: Seasonal
Rate of Pay: $2600 - $3000 stipend plus room, board, bedding, laundry, and recreational facilities provided.
