SCOPE AND PURPOSE OF POSITION:
The Care Manager provides comprehensive care management services to children and adults presumed to meet eligibility requirements of New York State Health Home program. They will gather documentation that confirms eligibility for the program and work with clients to assess and identify areas requiring supportive services. (Children’s Health Homes) The care manager works with children, families and service providers to complete the Child & Adolescent Needs and Strengths Assessment, which will guide them in creating a comprehensive Plan of Care. The care manager will collaborate with client’s service providers and with client/family to help achieve identified goals on Plan of Care. (Adult Health Homes) The care manager works with adults in creating a comprehensive Plan of Care. The care manager collaborates with the client’s other providers to help achieve identified goals on Plan of Care. Care manager will assess the need for additional services, such as Home and Community Based Services for both children and adults enrolled in the Health Homes program. The Care Manager will be required to document all contact related to client’s case in an electronic health record and monitors the need for additional interventions based on client need. This is a hybrid position.

The Social Ministries Regional Case Manager is responsible for the supervision and management of overall casework services offered to families through the Social Ministries programs at identified locations or social services/ministries site. The Social Ministries Regional Coordinator will work with the Divisional Social Ministries Director and collaborate with The ASSD and Divisional Pathway of Hope Coordinator to support the implementation of the program’s objectives including training of Corps staff involved in Case Management, collection of data, and reporting on outcomes. The principal function is to provide supervision to support the direct service, community resource development, and casework consultation to ensure the full implementation of Social Ministries, Strength Based programming and the Pathway of Hope model within the assigned Cluster.
QUALIFICATIONS
- Master’s degree in human service area: prefer LSW from an accredited college or university
- Prefer LCSW, LCP, or LCPC
- Minimum of four years of experience in the supervision of comparable social service programs that offer multi-faceted casework interventions designed to address the needs of families utilizing strengths-based approach.
- Experience and/or strong interest in community outreach, organization, and community capacity development.
- Must have an interest and ability to work with people and clients of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
- Ability to collaborate on complex social issues within families and communities.
- Ability to speak and understand English in a manner that is sufficient for effective communication with supervisors, employees, beneficiaries, and customers.
- Demonstrated capacity to teach adults.
- Familiarity with Salvation Army policies and procedures.
- Excellent oral and written communication skills.
- Travel, including overnight and weekends, will be required occasionally.

Our Eastern Territorial Headquarters in West Nyack, NY has 2 temporary part-time positions (up to 30 hours/week) for a Production Assistant supporting Live Events. These positions are expected to begin at the end of May and end in mid-July, 2024.
As a member of the Event Team, this creative and problem-solving individual will support all aspects of live events productions that convey the message of the Gospel of Jesus Christ. When not on set or on a live events production, this individual will support in preparation of materials, equipment and logistics.
This social ministry position is a position which requires a strong concentration in case management. The overall purpose of this position is to provide traditional Corps social services to all clients including food, shelter referral, clothing, and utility assistance.
The Family Service Worker will interview and evaluate clients determining various problems and needs. They will also provide emergency assistance to alleviate the immediate basic need for issues such as food insecurity, disconnection of utility services, and emergency shelter referral. The case manager will also provide referrals as needed to outside agencies.
Duties will include execution of department seasonal programs (Christmas, Camp NEOSA (overnight), and Back to School) as well other department programs/events, as appropriate.

SCOPE AND PURPOSE OF POSITION:
Pathway of Hope (POH) is family case management; an innovative, holistic, client-centered approach to helping families break the cycle of intergenerational and chronic poverty, effectively improving social determinants of health as well as enhancing quality of life and health outcomes. The POH strategy involves outreach, working with families in their homes and in the community; linkages to community resources to build, establish and maintain stability and self-sufficiency; and increase hope for a brighter future. POH Case Manager will be responsible for the provision of long term (up to 2 years) intensive case management services to families with minor child(ren), who are living in inter-generational, chronic poverty, and who have demonstrated interest or desire to take action. Caseload minimum of 16 families per year.

QUALIFICATIONS

- Vacuum, mop, and clean all floors as needed

- Clean glass doors and windows
- Remove all trash to dumpsters
- Monitor activity areas and clean after use

viii. Monitor and complete all minor repairs. Arrange with the corps officer any major repairs that need attention.
- Stripping and waxing floors twice a year and buffing floors when necessary.

- Check security of buildings as needed.
- b) Monitor outside property for cleanliness:
- Rake up leaves during the fall months keeping parking lot and entrances clear.
- Clear entrances, walkways, of snow in winter months.
iii. Change front sign when assigned by Corps Officer.
- Maintain cleanliness of area around the dumpsters.
- c) Monitor cleaning and maintenance supplies. Making sure all supplies are put away in storage closet and closet is locked. Work with Administrative Assistant to order supplies as needed.
- d) Driving as needed (bread runs/food bank, toy pickup/delivery)
- e) Maintain cleanliness of vehicles as assigned.
- f) Supervise community service volunteers assigned to assist in cleaning of the building.
- g) Perform other related tasks assigned by the Corps Officers

Reporting to the Corps Officers, the position gives hands-on oversight to and administration of the Northwest Corps Kids Summer Adventure Program. This is a 40 hour week position.
