This is a 143 –bed facility located at 681 Clarkson Avenue Building 8, NY. Provide the safety and the well-being of clients residing in the facility as well as compliance with all rules and regulations. This position assumes the operational management of the facility in the absence of another manager on site and responds to all emergencies. Functions of the job include but are not limited to the duties listed in the job description.
The Shift Supervisor manages the flow of people in and out of the facility and ensures the safety of all clients and staff as well as compliance with all facility rules and regulations. The Recreation Coordinator is responsible for the coordination of recreation activities for children after school and when school is not in session. The Recreation Coordinator plans, organizes and coordinates all youth and family programming.
The Peer Specialist will provide direct peer support services to clients, contribute to team planning, and assist with outreach and transitional care efforts in community settings. Most importantly, the Peer Specialist will draw upon his or her own lived experience to help develop and maintain the program’s culture of respect and focus on the client’s potential for returning to independent living. The Peer Specialist must possess the ability to engage and affect change through a person-centered model. The successful candidate will demonstrate initiative, flexibility, creativity and a collaborative spirit.
Responsible for coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, maintaining trip log for van use. The Maintenance/Handyperson is responsible to perform routine repairs in and around the facility Maintenance Worker /while adhering to a standard that meets all outside monitoring requirements to ensure a safe and clean environment for our individual’s homes. Additionally, the Maintenance/Handyperson is responsible for participating in the duties of the regular cleaning and sanitizing of the property. The Maintenance/Handyperson ensures the proper maintenance and upkeep of the facility.
· Services which Functions are related to DSPs (such as, but not limited to Maintains and submits timely and accurate nutritional records and reports; Assist each resident, in skill development in areas such as domestic living, medical appointment; Assist in transporting of the individuals to and from programs, community inclusion activities, medical appointments and etc.)will provide on a temporary basis when the DSP is not present for any reason.

Director of Nursing, Work as a full-time nurse and provide overall supervision to all nurses and programs regarding their all aspect of medical care in conjunction of OPWDD and The Salvation Army DDS programs. The Director will oversee a maximum of 96 Person Supported. They are also tasked with supervising and mentoring an average of five (5) nurses, including three full-time and two part-time nurses (or equivalent).
