Under the supervision of the group teacher, the teacher-aide shall be responsible for: WORKING WITHIN THE FRAMEWORK OF THE SALVATION ARMY of the day care center and   carrying out its functions, policies, and procedures.      To assist in the planning and execution of a program to provide day care for children which takes into account the educational, social, developmental and health needs of the group, as well as the individuals child. (Adult Health Homes) The care manager works with Adults in creating a comprehensive Plan of Care. The care manager collaborates with the client’s other providers to help achieve identified goals on Plan of Care. Care manager will assess the need for additional services, such as Home and Community Based Services for both children and adults enrolled in the Health Homes program. The Care Manager will be required to document all contact related to client’s case in an electronic health record and monitors the need for additional interventions based on client need.

The Program Director will work with the Staff in managing the program in adherence to the New York State School-Age Child Care regulations, as well as government contracts.   He/she will monitor and support his/her staff and implement student curriculum activities.  Familiarity with the student population served is essential.
