The Care Manager II is responsible for independently enrolling a minimum of 3 cases per month (36 cases a year); Provide follow-up services to meet the client’s needs and tracking for their caseload. Case load volume will be determined by the Administration of HIV, Health Home and Behavioral Services, adjustments to case load will be made according to agency need. Must use discretion and independent judgment in exercising duties. 

The Maintenance/Handyperson is responsible to perform routine repairs in and around the facility Maintenance Worker /while adhering to a standard that meets all outside monitoring requirements to ensure a safe and clean environment for our individual’s homes. Additionally, the Maintenance/Handyperson is responsible for participating in the duties of the regular cleaning and sanitizing of the property.
