The Salvation Army, Syracuse Area Services is seeking an FFT-CW Case Planner II to join our specialized Functional Family Therapy–Child Welfare program. In this role, you'll work directly with children and families to help strengthen relationships, address challenges, and help prevent foster care placements.

 

What You'll Do:

  • Work directly with 10–12 families using the evidence-based FFT-CW model.
  • Build strong, supportive relationships with children, caregivers, and families.
  • Meet with families in their homes and communities to address individual and family needs.
  • Develop individualized goals and service plans with families.
  • Identify strengths, address risk factors, and promote child safety and family stability.
  • Coordinate with schools, community providers, and child welfare partners.
  • Complete required documentation and progress notes.
  • Participate in specialized FFT-CW training and ongoing professional development.

What We're Looking For:

  • Master's degree in Social Work or a related clinical master's degree preferred.Will consider those nearing completion of degree.
  • Strong communication and relationship-building skills.
  • Ability to work effectively with children and families experiencing complex challenges.
  • Valid driver's license and reliable transportation.

Responsibilities

 

Key Responsibilities
  • Conduct comprehensive intakes, assessments, and ongoing evaluations to identify client and family strengths, needs, and service goals.
  • Provide FFT-CW services through regular client engagement, family therapy sessions, and required contacts to support family stability and positive youth outcomes.
  • Develop and implement individualized service plans in collaboration with families, Department of Social Services (DSS), and community providers, using FFT-CW strategies to promote behavioral change and family preservation.
  • Participate in FFT-CW training, supervision, consultations, and case conferences while maintaining clinical assessments, outcome monitoring, and program documentation.
  • Deliver case management, counseling, crisis intervention, advocacy, referrals, and service coordination to address client and family needs.
  • Complete timely and accurate case records, reports, assessments, narratives, statistical data, and recommendations in accordance with program standards.
  • Maintain strong partnerships with DSS and community agencies, provide case recommendations, and demonstrate cultural competence in all aspects of service delivery.

Qualifications

Qualifications

  • • Master’s Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or related clinical Master’s Degree is preferred. Will also consider candidates nearing completion of one of these degree programs.
    • Licensure is preferred
    • Must have access to reliable transportation and valid license that meets The Salvation Army Insurance Requirements.
