SCOPE AND PURPOSE OF POSITION:
The Care Coordinator Supervisor is responsible for the supervision of assigned staff and is accountable for adherence to regulations and policies of The Salvation Army, Lead Health Home and NYS DOH. Supervisor will support staff to integrate models of care with the intention of providing whole family case management.
The Care Coordinator Supervisor will be responsible for a reduced caseload of Pathway of Hope and/or Health Home Care Management cases and will support program growth in assigned region. Position requires regular, weekly travel for case management and team supervision.

Position Overview:
Assist with planning, preparing and the implementation of Senior Program activities
The Case Manager provides supportive and direct intensive case management services to the homeless families residing in the Shelter. The Case Manager assesses the client’s needs and jointly develops a collaborative plan that addresses those needs. The Case Manager prepares the client for self-sufficiency and independent living and supports their transition back into the community. The Case Manager engages all clients and conferences with them when issues of noncompliance are identified. The Case Manager organizes case conferences and ensures that their clients are actively involved in their discharge planning.
The Social Worker is responsible for the overall delivery and coordination of services for the homeless population residing at Kingsboro Mica Shelter. The Social Worker is responsible for interfacing with the New York City Department of Homeless Services Staff, shelter providers and relevant community-based organizations to enhance and ensure clients are in receipt of optimal care and service coordination. They are responsible for leading a Clinical team made up of 3 case managers, 3 residential aides, and 1 peer specialist.
