POH Case Manager will be responsible for the provision of long term (up to 2 years) intensive case management services to families with minor child(ren), who are living in inter-generational, chronic poverty, and who have demonstrated interest or desire to take action. In addition, this position will work to advance AEI initiative by coordinating access to the Dept of Social Services’ childcare subsidy and acting as Facilitated Enroller to increase access to childcare resources and improve employment opportunities for parents and caregivers.
