Working directly under the supervision of the GAINS Program Director, the Assistant GAINS Program Director will assist with the oversight of all programmatic aspects of the year-round Bushwick GAINS Program. The Assistant GAINS Program Director will have direct responsibility for health & safety protocols and programming.
Under the supervision of the Corps Officer and Case Worker, the cook is primary responsible for preparing and serving all food in the center and for maintaining the kitchen in a sanitary and efficient condition. Additionally, the cook is responsible for the supervision and training of other staff assigned to kitchen or food service responsibilities. Follow individual work schedule provided by the supervisor to indicate specific responsibilities and the time scheduled for them. Cooperates in total staff effort designed to improve the center’s services. Participates in periodic individual conferences, trainings, workshops, certifications, and regular staff meetings as scheduled by the supervisor. These will be called when changes are planned in the soup kitchen program which affect the cook so that discussions and suggestions for improving the services may be shared among the supervisor and staff. Such meeting will also be called when policy or changes are be made affecting budgetary needs for food and supplies or rearrangement of space.
Participation in staff parties and attendance is parents meeting are also encouraged but not required, as a means of promoting a feeling of staff unity and proving opportunities for information exchange of information between parents and total staff.
The position as a Senior Center Attendant, will always be on hand to address the needs of the seniors and will be supervised by the Senior Center Director and Assistant Director. Two attendants will be needed, one female and one male. Employee will work 25 hours a week, from 9am to 2pm. One 15-minute break – one in the morning and one in the afternoon.

SCOPE AND PURPOSE OF POSITION:
TheCare Manager provides comprehensive care management services to children and adults presumed to meet eligibility requirements of New York State Health Home program. They will gather documentation that confirms eligibility for the program and work with clients to assess and identify areas requiring supportive services. (Children’s Health Homes) The care manager works with children, families and service providers to complete the Child & Adolescent Needs and Strengths Assessment, which will guide them in creating a comprehensive Plan of Care. The care manager will collaborate with client’s service providers and with client/family to help achieve identified goals on Plan of Care. (Adult Health Homes) The care manager works with Adults in creating a comprehensive Plan of Care. The care manager collaborates with the client’s other providers to help achieve identified goals on Plan of Care. Care manager will assess the need for additional services, such as Home and Community Based Services for both children and adults enrolled in the Health Homes program. The Care Manager will be required to document all contact related to client’s case in an electronic health record and monitors the need for additional interventions based on client need.

Pathway of Hope (POH) Case Manager will be responsible for the provision of intensive case management and Rapid Rehousing services to individuals and families with children under the age of 18, who are experiencing domestic violence. This is an outreach position working with families while in shelter, in their home as they move to safe housing and in the community.

Membership Service Lead responsibilities include but not limited to: selling and/or verifying memberships through POS system, providing information and answering questions about available programs. This position assumes a leadership role for the Customer Service Associate role and takes on certain responsibilities in the absence of the Sales Manager.
