The agency has embraced the concept of a fully integrated system of care for all individuals of service. This requires that all staff assesses the individual's needs holistically and see it as their responsibility to connect the individuals with services needed-internal or external to the agency, regardless of which of the individuals enters the system. Assess the needs of the whole individuals/family including their basic needs such as food, clothing and shelter. Ensure that referrals are made to appropriate internal and external services as required to meet the individual's families' needs. Follow-up on referrals you make on behalf of an individual’s family and respond to all referrals made to you/your program. Participate in cross-program service reviews that support the planning and coordination of service delivery and service closure and Other Duties as assigned.

The Housing Specialist assists homeless families in their pursuit of permanent housing. The Housing Specialist manages relationships with brokers, landlords, NYC housing, supportive housing, and any and all other housing options so that a steady stream of housing referrals can be accessed and provided to the families residing within the shelter. The Housing Specialist coordinates trips into the community to view available apartments, accompanies clients for intakes into different housing, and/or provides the clients with the means for viewing all housing options. The Housing Specialist conducts housing workshops in the program on the areas of program education, interviewing with a landlord, paying your bills, and any other topics deemed necessary to support movement and long-term placement into the community. The Housing Specialist works together with the case management team to ensure all clients are actively engaged in their own exit strategy and discharge plan or is referring the client for next step or involuntary transfer.

The Resident Aide ensures that the residents’ basic needs are met, and that the daily operations of the program are instituted in a manner consistent with the philosophy and policies/procedures of the program and agency. The position provides awake staff coverage for all shifts of the residential program, monitoring residents and assisting in clerical/data and billing reporting responsibilities, household management tasks including facility cleaning, laundry and other duties as needed.

The Social Worker is responsible for the overall delivery and coordination of services for homeless families residing in family tier II shelters and/or family hotels. The Social Worker is responsible for interfacing with the New York City Department of Homeless Services Staff, shelter providers, and relevant community-based organizations to enhance and ensure families are in receipt of optimal care and service coordination. The Social Worker is responsible for the completion of psychosocial assessments of complex families impacted by multiple systems. The Social Worker will work with these systems and facilitate appropriate linkages for families to community-based services. The Social Worker must be able to work from a multi-disciplinary team approach to service provision. The position calls for an individual able to work within a crisis driven environment with a knowledge base of substance abuse and addiction, domestic violence, child maltreatment, trauma, and mental health conditions. The Social Worker will provide education to other members of the team around clients’ psychosocial stressors and needs, to ensure positive outcomes for families. The Social Worker will obtain ongoing professional development by the individual agency program, and the New York City Department of Homeless Services – Division of Family Services. The Social Worker will be expected to provide qualitative and quantitative feedback to the Department of Homeless Services (DHS) and DHS Service Providers to inform both policy and practice.
