SCOPE AND PURPOSE OF POSITION:
The Case Manager position exists to assist in providing services for the Domestic Violence/Rape Crisis Program of The Salvation Army Anew Center in Chautauqua County. Based at The Salvation Army’s South or North County Office, the Case Manager will be responsible for the implementation of domestic violence, sexual assault, and rape crisis services throughout Chautauqua County. Interview clients seeking assistance for various human service needs and making referrals to other area agencies for further services. Maintain daily records of clients including but not limited to forms/files/correspondence/statistical information. Support Corps in the implementation of social ministries programs and collaborate with other social service agencies in the community.

QUALIFICATIONS
High School Diploma, required
Associate degree in Social Services, preferred
- Working knowledge of Microsoft applications (Word, Excel, PowerPoint, Access)

- Good problem-solving skills
- Knowledge of the local community and services provided
- Valid driver’s license and use of official vehicle may be required. Ability to travel as needed

SCOPE AND PURPOSE OF POSITION:
Care Manager 3 (high acuity) provides outreach and enrollment services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program. They gather assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work with children, families and service providers to create a comprehensive Plan of Health Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. Care Manager 3 documents progress and arrange for eventual transition between health homes as necessary, or from health homes to other levels of service following changes in eligibility.

The Evangeline Booth Miracle Home is an 18 bed shelter that provides emergency housing to homeless women and families of women and children. The Resident Aide provides emotional support, information and referral to community services, and creates a warm and safe environment for the residents. The Resident Aide ensures that the residents’ basic needs are met, and that the daily operations of the program are instituted in a manner consistent with the philosophy and policies/procedures of the program and agency. Provide awake staff coverage of residential program, monitoring residents and assisting in clerical work, household management tasks and other duties as needed.

This is a 143 –bed facility located at 681 Clarkson Avenue Building 8, NY.
