Case Management Program Manager
Scope and Mission
The Salvation Army of the Empire State Division provides life changing programs and services. Programs consist of; housing and homeless facilities, children and adult care management, food insecurity, anti-human trafficking outreach and case management, after school/youth enrichment centers, and a national evidence based intergenerational poverty elimination strategy, Pathway to Hope.
Essential Duties and Responsibilities
The Case Management Program Manager is responsible to assure the provision of high-quality, person-centered service that is in accord with the policies and regulations of the Salvation Army, the NYS Health Home Care Management Program and the evidence-based National Pathway of Hope Strategy. This professional position will be responsible for developing new referrals, engaging cases, and the support of program expansion across the forty-eight upstate counties in New York. The position collaborates with the director to provide case supervision and support to staff in the field. Role includes community engagement, operational activities, program supervision, and the monitoring of overall quality goals and compliance.
This position is a part of the Salvation Army Empire State Social Services team located in Syracuse; however, this is a field-based position traveling through the geographic service area.
Experience and Skills Required
Candidates with a master’s degree in related field with 2 years of successful work in multifaceted human services programs preferred or a bachelor’s degree with 3-5 years of direct service experience. Proven record in collaborations, partnerships, and community engagement, with understanding of the dynamics of intergenerational poverty and social determents of health and their impact on the community we serve.

This position offers competitive compensation commensurate with this key leadership role.
