SCOPE AND PURPOSE OF POSITION:
The Case Manager Supervisor position exists to supervise Case Managers who provide services for the Domestic Violence/Rape Crisis Program of The Salvation Army Anew Center in Chautauqua County. Based at The Salvation Army’s South or North County Office, the Case Manager Supervisor will have overall responsibility for the implementation of domestic violence, sexual assault, and rape crisis services throughout Chautauqua County. The Case Manager Supervisor is part of the Leadership Team.
Completes intakes, assists with discharges, completes paperwork as needed. Works closely with adult residents. Provides orientation to the program. Reviews program rules, policies and procedures with clients. Reports rule violations, injuries, and incidents to assigned worker and management – both verbally and in writing via the special report (must be completed by the end of their scheduled shift).
Rate of pay is $16.00/hr

Availability of Position: Part time Plus - 25 hours a week

QUALIFICATIONS
To assist the BTG Coordinator with the Bridging the Gap program
- Help plan and assist Bridging the Gap Curriculum
- Review lesson plans and offer input
- Attend weekly staff meetings
- Attend at least one group per week as an observer
- Discuss each participant’s progress with BTG Coordinator and interns
- Maintain a document of all participant’s addresses
- Advocate for participants where necessary.
- Talk to participants as required/needed
- Handle and teach teens at Bridging the Gap with varying personal backgrounds
- Attend training sessions as required
- Coordinate with area colleges and graduate programs to recruit interns and volunteers.
- Represent The Salvation Army’s social mission in the Bridging the Gap program

- Perform other related tasks as assigned by supervisor

The Case Manager provides supportive and direct intensive case management services to the homeless families residing in the Shelter. The Case Manager assesses the client’s needs and jointly develops a collaborative plan that addresses those needs. The Case Manager prepares the client for self-sufficiency and independent living and supports their transition back into the community. The Case Manager engages all clients and conferences with them when issues of noncompliance are identified. The Case Manager organizes case conferences and ensures that their clients are actively involved in their discharge planning.
Case Manager at Kingsboro Men’s Shelter MICA Program. This is a 143-bed facility located at 681Clarkson Avenue Brooklyn NY 11203. The Case Manager renders direct casework and/or group work services to residents and is responsible for assessing, referring, and follow-up of social services.
Employee reports directly to the Facility Manager. The Porter is expected to maintain the appearance of the building clean and orderly and generally serving the residents.
