Overview

This position will provide intensive, home-based case management to approximately 72 families per year. Services include intake, assessment, data entry, housing search and advocacy, life skills training, case plan goal development, and referral and linkage to community resources. The Family Worker will provide support to childcare teachers and families, as well as support child development standards.

Responsibilities

  • Meet clients (childcare center families and students) in their homes, as needed.
  • Prepare case management-related reports, including but not limited to outcomes, successes, etc.
  • Assist clients in obtaining necessary resources to achieve case management goals.
  • Manage early childhood developmental screening and assessment tool, including follow-up visits with teachers and families for referrals and action plans.
  • Work with families for initial interviews and exit interviews, as well as to address any concerns, questions, and/or complaints.
  • Track and monitor child absenteeism.
  • Support the Strengthening Families Program.
  • Support Teacher with Teaching Strategies/Creative Curriculum Ready Rose tools and oversee engagement with parents.
  • Conduct a minimum of 2 scheduled home visits for all students in the program.
  • Oversee GOLD and Brigance System to support teachers in the process.
  • Support teachers with children filing processes.
  • Support Childcare Director with the registration processes.
  • Refer children and families to appropriate early intervention resources.
  • Prepare monthly report to Childcare Director related to family visits and open cases.
  • Work with teachers for open cases.
  • Be a liaison between families and the school district.
  • Coordinate food distribution for families as needed.
  • Perform outreach to the community for enrollment and community engagement.
  • Coordinate and support events for the community, Corps, and childcare families in collaboration with the Program Director and Childcare Center Director.
  • Prepare files, reports, and statistics for the Childcare Director as needed.
  • Prepare and maintain staff record for trainings assigned by state regulations, Grow NJ Kids, District, and childcare goals.
  • Interview, assess, and collect client data, collaborating with Corps Ministry Teams and community partners, to formulate an appropriate plan of action to move the client families from crisis to stability. Work with clients who are involved with other social service systems and adults in crisis.
  • Assist clients requesting hotel vouchers, energy, and rent assistant.
  • Manage social programs as needed and update information to the service point system
  • Assist on the maintenance of the main office and main entrance.
  • Manage community bulletin boards with updated information.
  • Coordinate volunteers.
  • Other duties as assigned.

Qualifications

  • Associate or bachelor’s degree in Social Work, Sociology, Human Behavior, or Education preferred.
  • At least 2 - 5 years of experience in case work, social services, community development or related field required.
  • Experience working with low-income families.
  • Knowledge of social services programs offered by the county and state.
  • Bilingual (English/Spanish) required.
  • Strong work ethic.
  • Strong written and verbal communication skills.
  • A positive, contagious, “can-do” attitude.
  • Able to maintain strict confidentiality.
  • At least 21 years old with a valid New Jersey Driver’s license and the ability to obtain and maintain driving privileges per Salvation Army insurance standards.
  • Must understand and support the mission of The Salvation Army.
