    Department/Location:   Supportive Housing, Reading, PA Corps

    Availability of Position: Immediately

    Schedule: Full-Time; Monday-Friday 8:00 AM - 4:30 PM

    Reporting Relationship: Supportive Housing Supervisor

    Salary and Grade: $14.85-$22.25 per hour; 75 Non-Exempt

    Position Overview:

    Responsible for providing all necessary case management services and referrals to families and individuals participating in our Supportive Housing Program, provide housing locator services and perform outreach services to marginalized persons.  Represent TSA on community panels and coordinate service efforts with partner agencies.

     

    Responsibilities

    1. Assist participants to find suitable housing
      1. Ensure properties meet established standards, perform rent calculations, process rental agreements and process rent/utility payments
      2. Negotiate rental rates to ensure compliance with HUD Fair Market Rental Value limits.
      3. Act as a housing locator on behalf of all housing department staff and programs.
    2. Support case work staff regarding professional social work procedures, case management duties, program management and implementation in coordination with the social services director and corps officers.
    3. Maintain case records on each participant including assessments, social history, service plan, progress notes and other supporting documentation as appropriate / directed 
    4. Conduct case management interviews with participants both in-home and in the office
    5. Focus on the employment needs of participants.
      1. Assistance with job search including resume development, employment application assistance, practice interviews and post interview debriefings
      2. Coach clients regarding workplace issues including good working relationships with supervisors and peers, time management, and enhancing skill levels
    6. Referrals to community-based services
      1. Mental health, substance abuse treatment services, etc.
    7. Responsible for ensuring that program participants have access to mainstream health and social service programs such as TANF, Medicaid, CHIP, SSI/SSD, Food Stamps, Work Force Investment Act, and Veteran’s Health care program.
    8. Assist participants to further enhance personal social support systems
      1. Community activities, children’s schools, faith communities, etc.
    9. Assist in presenting life skills training weekly (parenting, budgeting and personal finance, cooking/nutrition, conflict management, substance abuse, job readiness/search training, etc.
    10. Maintain client record using HMIS system
    11. Participate in community meetings and programs
    12. Represent The Salvation Army by serving on community social service outreach (Community Connections Project, QI committee, homeless coalition, etc.) in coordination with the social services director and corps officers.
    13. Assist clients in coordination of the move-in process as needed
    14. Administer the HUD Rapid Re-Housing program (interview & place clients through the case review process, provide case management and property location assistance, coordinate the internal management processes (rent payment, reporting statistics through HMIS and internal TSA processes as needed, etc.)
    15. Performs related duties as assigned.

    Qualifications

    • MSW; or BSW with related work experience, or equivalent education and experience in a related field.
    • LCSW preferred, but not required
    • Computer proficiency in Microsoft Excel, Word, PowerPoint – ability to learn HMIS, etc. as needed in the normal course of performing job duties
    • Hands-on experience developing case records, doing case interviews, and case management
    • This position requires an ability to learn about community resources and to share that information with program participants
    • Must possess good interpersonal skills, and have a willingness to work with disadvantaged families and individuals
    • Bilingual (English and Spanish) preferred, but not required
    • Child Protection Clearances PA Act 153 requires that all prospective employees complete the following clearance and background checks prior to employment:   PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record/Fingerprint Check. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check along with renewals every five years.
