  • Overview

    Under the direct supervision of the Program Manager, the Case Manager provides case management services to homeless residents/clients during their temporary stay at Booth Haven/Safe Haven Men's shelter.  

     

    Responsibilities

    1. Assess and identify resident’s/clients' needs. Advocate for residents/clients to ensure that services of referral are accessible and provided to them.
    2. Develop an individualized service plan for each resident/client and ensure a weekly review of goals.
    3. Provide supportive and crisis counseling and guidance both individually and to groups.
    4. Provide information and refer residents/clients to relevant community social services for medical, mental health, substance abuse, educational, vocational, public assistance, etc.
    5. Maintain all records and documents relative to the interview/counseling meeting and subsequent progress/status reports in accordance with agency and contract requirements.
    6. Develop and maintain collaborative working relationships with community groups and organizations to enhance quality of service to residents/clients.
    7. Report observations to supervisor on trends of the homeless, their service needs, effectiveness of referrals.
    8. Daily maintenance of HMIS data entry (intakes, services, discharges, case notes, etc.) and other internal data collection tools.
    9. Compile monthly statistics, and prepare required reports in a timely manner.
    10. Conduct Case Management related client groups either directly or co-facilitate with community organizations.
    11. Share rotating 24hr. on-call responsibilities with the Program Manager, Booth Haven and Safe Haven Senior Case Manager and Senior Resident Assistants once every three weeks. 
    12. Attend staff meetings. As requested, attends in-service conferences, seminars, training programs, and other agency informational meetings.
    13. Share office coverage at least 1-2x a week during lunch time.
    14. Other duties as assigned.

     

    Qualifications

    Bachelor’s Degree in Human Services with 2 years case management experience required. Requires experience working with and providing advocacy for vulnerable populations, as well as an understanding of barriers faced by those who are chronically homeless. Strong computer skills. Ability to collaborate effectively with external service providers. Must possess a knowledge of social service resources and strong organizational abilities with attention to detail. Valid NY State Driver License.
