    REPORTS TO:          Adult Day Services Director


    SUPERVISES:                       NA                              


    STATUS:                   Full Time, Permanent, up to 40 hours per week                   


    SCHEDULE:                          Must be available Monday through Friday 7 am to 5 pm


    FUNCTION:   Responsible for the general supervision of clients, providing personal care to the clients, implementing planned activities, and the cleanliness of the adult day program.


                            Responsible for safely transporting participants to and from the program, escorting adult day service participants to and from vehicles, providing general supervision of participants and the documentation related to the transportation program.            



    • Provide supervision and personal care needs of the participants, including, but not limited to assistance with ambulating, mobility, eating, grooming, and toileting
    • Assist Activity Coordinator with planning and implementing daily and monthly activities
    • Assist Activity Coordinator in documenting client participation in activities
    • Assist with the implementation and documentation of Individual Service Plans as required
    • Observe and report any change in participant status to the nurse and/or director
    • Assume appropriate team responsibilities to meet program needs
    • Assist with training and directing volunteers, including, but not limited to Mature Service workers and Job and Family Service-Work Experience Program workers
    • Assist with meal site tasks, including, but not limited to meal orders, set up, and clean up
    • Participate in at least twelve hours of in-service or continuing education on appropriate topics each calendar year
    • Maintain CPR and First Aide Certification
    • Perform daily vehicle inspections using the appropriate checklist prior to the first route of the day. Report any problems or concerns found
    • Perform monthly vehicle inspections using the appropriate checklist at the beginning of each month
    • Correctly and neatly record required information on the transportation log
    • Assist the participants to sign in on their own attendance page to ensure that participants arrival and departure time is recorded as appropriate on the attendance page
    • Check the van list/route sheet for van route and participants to pick up or take home
    • Assist in reviewing and writing the van list/route sheet as needed to ensure the most efficient way while meeting the time needs of the participants
    • Keep driver cell phone charged and ready to use at all times
    • Ensure the driver cell phone is available during the routes and use to communicate route attendance changes
    • Must complete within six months of hire and every three years a DRIVE training and Defensive Drive training as required by funding sources
    • When necessary, perform janitorial duties related to the transportation program. For example, sweep/vacuum/mop the vehicles as needed; clean windows as needed; sweep, shovel, and salt sidewalks and patios as needed
    • Be conscientious of safety related matters throughout the building, on the property, and especially regarding the vehicles
    • Coordinate vehicle maintenance for Samaritan Center vehicles
    • Other duties as assigned by the Program Director, Activity Director, and/or Corps Officer


    • Must be a high school graduate, or have successfully completed a GED
    • Must have a minimum of two years employment experience in providing personal care activities and/or social/recreational services under the direction of a licensed or certified health care professional
    • State Tested Nursing Assistant Certification preferred
    • Must pass Criminal Background Check with no disqualifying offenses
    • Must be CPR and First Aide certified
    • Must be in good health
    • Must be able to lift up to 50 pounds
    • Must be capable of performing routine cleaning tasks
    • Must be reliable and dependable
    • Must have a signed statement from a licensed physician stating the driver has no medical or physical condition that may impair safe driving, passenger assistance, emergency treatment, or the health and welfare of the participant or general public
    • Must pass drug and alcohol tests
    • Must possess the ability to understand written and oral instructions
    • Must be at least 21 years of age
    • Must possess a valid Ohio Driver’s License with at least 2 years driving experience
    • Must have an understanding of and appreciation for the Mission of the Salvation Army
