    Position: Bridge Case Worker- New Castle County/Kent/Sussex Counties

    Department:  Bridge Program, Delaware State Office                

    Reports to:   Social Services Director

    Availability:  September 1, 2018 (4 positions available)                          

    Schedule:  Full time – Generally 8 a.m. – 4 p.m.

    Salary and Grade: $12.00 per hour; 73 Non-Exempt

    Job Summary:

    The Bridge Case Worker will be responsible for assisting participants that have barriers to employment by providing case management services to individuals enrolled in the State of Delaware’s TANF program or Wonder program as part of the welfare to work employment and training initiative.  The ideal candidate will work together with the participants and other E & T vendors to provide comprehensive case management services to alleviate barriers to self-sufficiency by assessing the participants strengths as well as barriers in the following areas: living environment, risk of homeless ness, substance use severity, mental and physical health, history of victimization and access to treatment services.

    Responsibilities

    • Identify barriers and set goals to address barriers.
    • Arrange, coordinate and link clients to current resources.
    • Establish a nurturing, trusting and caring environment
    • Monitor program participation to ensure clients are on track to achieve goals.
    • Complete Wonder clinical case reviews.
    • Provide case management services.
    • Must have an understanding of and appreciation for the Mission of The Salvation Army.
    • Adhere to all policies and procedures.

    Qualifications

    • Associates degree preferred in clinical or counseling psychology, social work, or human services field or 2 years comparable work experience.
    • Desire and commitment to support high risk, emotionally, mentally or physically challenged persons in their goal of becoming self sufficient.
    • Bi-lingual a plus.
    • Experience working with people of diverse socio-economic groups and cultures.
    • Strong relationships with local social service and community agencies.
    • Ability to manage multiple projects concurrently and apply priorities appropriately.
    • Strong written and verbal communication skills.
    • You must be able to obtain clearances required by the Joseph R. Biden III and the Child Protection Act.
