    Position: Case Manager (CM)

    Department: Philadelphia Social Service Ministries, New Day Program, Philadelphia, PA                

    Department:   Philadelphia Social Service Ministries – Reed House 1320 S 32nd St, Philadelphia

    Availability of Position: Immediately; BSW grads encouraged to apply.

    Schedule: Full Time; flexible schedule to include some weekend and evening hours

    Reporting Relationship: Case Manager Supervisor

    Salary and Grade: $33,500 per year; 10 Exempt

    The Case Manager (CM) provides comprehensive social services to adults who live with a disability and have experienced homelessness and currently reside in a permanent supportive housing program.  The CM is responsible for assessment, service plan development, linkage and regular contact with participants to assess progress. Additionally, the CM provides counseling to clients and is responsible for HMIS electronic documentation and file maintenance.  The above list of job duties is not exclusive or exhaustive and the candidate will be required to undertake such tasks as may reasonably be expected within the scope of the post.


    Must have a College Degree in Social Work or related field. 1year of case management experience is a must or supervised SW internship.  Individual must be well organized, detail oriented, computer literate, willing to work in a trauma informed environment and  be able to relate to and work with multi-diagnosed individuals. Must maintain and uphold the principles and commitments of The Sanctuary Model. Child Protection Clearances PA Act 153 requires that all prospective employees complete the following clearance and background checks:   PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record/Fingerprint Check. Candidate must have an appreciation for, and understanding of, the Mission of The Salvation Army.
