    FLSA Status: Full-time, Exempt

    Hours Per Week: 40

    Schedule: 8:30 am - 5:00 pm (flexible), Mon-Fri, on-call two weeks per month


    Scope & Purpose of Position:

    Provide direct services, including case management and other services for which a client is qualified, to victims of human trafficking in the Greater Hartford area. Serve as the primary point of contact for survivors of human trafficking in coordinating necessary services and resources to address the survivors’ complex needs, including but not limited to emotional and mental health, housing/shelter, employment, medical, legal, and other supports. Participates in program planning, community education and outreach, and involvement in local and statewide task forces, coalitions, and councils. Supervises Outreach Worker and the outreach efforts of the program. Operate within program budget and funds; serve as a liaison for the program; and fulfill all administrative requirements of the program, including submission of progress reports and data entry.

    Case Management

    • Provide comprehensive case management services to survivors of human trafficking that follows a trauma-informed, harm reduction, and survivor-centered approach.

    • Provide survivors with an initial assessment and proceed with safety planning, orientation to services, support, and referrals by evaluating individual needs.

    • Collaborate with survivors to develop a service plan and timeline based on identified goals and barriers.

    • Assist survivors with accessing resources including shelter, emotional and mental health, medical services, employment services, legal services, and others as determined by the individualized service plan.

    • Maintain weekly contact with survivors through phone calls, emails, home visits, etc.
    • Meet face-to-face with survivors throughout the month to review goals and make any updates or changes.
    • Accompany survivors to appointments, social service agencies, meetings with law enforcement, court, etc.

    • Prepare and maintain quality case notes and documentation for any contact made and service provided.
    • Track program statistics and outcomes, enter data, and draft and submit reports as requested.
    • Attend relevant trainings, seminars, conferences and other professional development opportunities as instructed.
    • Attend staff meetings monthly and case review meetings biweekly along with Hartford Citadel Corps Officers.
    • Attend all anti-trafficking councils, committees, and community meetings as appropriate to include, but not limited to; Committee for Hartford Area Against Trafficking (CHAAT), Trafficking in Persons Council, Youth Engagement Team Initiative (YETI), and Human Anti-Trafficking Response Team (HART).
    • Refer and connect clients to the appropriate Corps locations for the following; pastoral care, Celebrate Recovery, and Corps adult & children’s programs.
    • Engage and support the community by participating in outreach efforts to increase human trafficking awareness and victim identification including making presentations on the program and human trafficking issues.
    • Conduct and participate in volunteer training and coordination as needed.

    Staff Management

    • Supervise the Outreach Worker; conduct formal one-on-one supervision at least monthly; conduct hiring and termination of position; complete annual evaluations as well as probationary review.
    • Ensure ongoing staff development opportunities, i.e., training through orientation, staff meetings, workshops, attendance at professional meetings/seminars, and maintenance of job description for position.
    • Oversee the development and implementation of an outreach plan that involves community awareness campaigns and street outreach efforts.
    • Oversee the recruitment, training, and coordination of volunteers to assist with outreach.

    • Bachelor's degree in social work or a related social services field; two years of case management required.
    • Prior experience of at least two years working with survivors of human trafficking or other victims of crime preferred. Experience should include providing direct service to underserved populations. Survivors of human trafficking are encouraged to apply.
    • Linguistic and cultural competency to holistically serve survivors of human trafficking.

    • Computer proficiency required.
    • Knowledge of, or willingness to learn, a trauma-informed, harm reduction, and survivor-centered approach.

    • Bilingual in English and Spanish a plus.

    • Excellent organizational and case file/documentation management skills.
    • Working knowledge of the functions of community organizations, social services, public benefits, health care systems, and related human services.

    • Ability to communicate effectively, both verbally and in writing.
    • Experience in public speaking and/or speaking to small groups preferred.

    • Excellent interpersonal skills including empathetic listening.

    • Must have a valid driver’s license and clean driving record.

    • Ability to maintain ethical conduct in accordance with organizational policy, and carry out business in a manner consistent with the mission, values, and policies of the organization.

    • Ability to work independently on assigned duties and willingness to seek and accept supervision as needed.

    • Unquestioned confidentiality with sensitive personal information.
