Availability of Position: 2 positions available immediately
Schedule: Monday-Friday; Requires some evening hours.
Reporting Relationship: Director
Salary Grade: 10 Non-Exempt
Description of Position:
The Case Manager is responsible for resident assessment, service plan development, regular contact with the residents to assess progress and needs, provision of guidance and counseling to clients as needed, making referrals to social services not provided at the shelter, preparation of applications for housing, documentation of resident progress and of services, ensuring completion of required forms, staff consultation, and other duties as assigned. The Case Manager uses the mission and vision of The Salvation Army, as well as The Sanctuary Model, as guiding principles in all aspects of the position.
