Housing Case Manager - Hartford No Freeze Program

Status: Non-Exempt, Part-time, Temporary

Hours Per Week: 20 hours

Schedule:  Monday – Thursday, 4:00 pm - 9:00 pm 


Assist clients in moving from homelessness to permanent housing through providing direct case management services. The Case Manager is responsible for working one on one with each household to identify their obstacles to permanent housing and to develop action plans to address and overcome those barriers to achieving self-sufficiency. The Case Manager serves as a link to community resources and service providers.

  • Follow up with clients within one business day of intake and schedule appointment to complete assessment.

  • Complete assessment with participants within two business days of admission.

  • Work with each client to develop an individualized goal plan addressing the individual needs of the client including: income sources, mental health needs, potential housing resources, linkage to community benefits, family dynamics and improvements.

  • Review client files and logbook daily and utilize this information to address unresolved situations with participants and partner with the Program Coordinator to identify possible resolutions.

  • Screen clients by completing the VI-SPDAT/ Next Steps assessment to determine possible eligibility and refer to appropriate housing programs.

  • Compile and complete statistics for The Salvation Army, HFPG, and City of Hartford.

  • Connect clients to clinical services for a mental health/substance abuse assessment when indicated and ensure that services are being received through documentation in the client file.

  • Attend all staff meetings and provide comprehensive case reviews.
  • Attend all GH-CAN meetings and maintain positive relationships with community housing providers.

  • Attend weekly case management meetings.

  • Utilize training and skill set to provide crisis intervention services, helping clients to remain calm and identify next steps.

  • Maintain regular weekly communications and work in harmony with the Program Coordinator to adequately track client progress.

  • Document all client interactions, goal progress and referrals in timely and concise case notes.
  • Maintain all intakes, discharges and bed nights in HMIS for Warming Center and hotel clients. Run a monthly utilization report for Hartford No Freeze program maintaining, at minimum, 95% data quality.

  • Close participant files using outlined Exit Process.

  • A Bachelor’s degree in Human Services or related social services field preferred and a minimum of two years of experience in human services which includes one year involvement with the homeless

  • Knowledge and proficiency in Microsoft Word, Excel, and Microsoft Windows based programs

  • Need to possess a valid driver’s license and reliable transportation.

  • Bilingual in Spanish preferred.
