Position: Pathway Of Hope Case Manager

Department: Philadelphia Social Service Ministries; 4050 Conshohocken Avenue, Philadelphia

Availability of Position: Immediately

Schedule:  Full Time - Monday through Friday from 8:30 A.M. to 4:00 P.M.

Reporting Relationship: Director of Philadelphia Social Service Ministries

Salary and Grade: $16.75 per hour; 75 Non-Exempt

Description of Position: 


The Pathway Of Hope Case Manage provides supportive social work services to a small caseload of families who are actively seeking support and guidance on their path to self-sufficiency.   This worker operates under a strength-based, trauma-informed approach to interview potential applicants, determine eligibility, develop case plans for self-sufficiency, and provide linkage and advocacy order to access appropriate services for successful goal acquisition.


Minimum Requirements:  


Must have a minimum of a Bachelor’s degree in a social services related field, Bachelor’s degree in Social Work preferred with previous case work or social service experience. Experience maintaining case records, facilitating sessions and case management. Computer literate and experienced with Word and Excel. Must have current PA driver’s license and access to a vehicle. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of  employment with recurrent training required every three years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check. An appreciation for, and understanding of, the philosophy and mission of The Salvation Army is required.
