Overview

Advocate on behalf of individuals with a developmental disability with the focus on Person Centered Planning. Assist the individual to acquire and maintain service in his/her community.

 

Responsibilities

  • Advocate for individuals with a developmental disability.
  • Assist the individual to acquire services in his/her community
  • Perform face to face visits or monthly telephone contact as required by OPWDD
  • Ensure that the individual maintain his/her benefits and entitlements
  • Develop and maintains the Individualized Service Plan in accordance with OPWDD
  • Reviews PEP, MMA and Habilitation Plan at least semi-annually
  • Write case notes to reflect the services rendered to the developmental disabled
  • Acquire necessary documents for the individual’s files to ensure continuation of the individual’s services and plan of care.
  • Maintains the consumer's primary record.
  • Maintains contact with consumers as identified in ISP
  • Ensures the completion and annual redetermination of the need for the level of care Medicaid eligibility.
  • Documents service coordination activities in service coordination monthly notes.
  • Facilitates and documents a change in service coordination when requested by the individual& advocate.
  • Secures an advocate for each individual if they require or choose to have one.Maintains an up to date monthly roster of individuals receiving services as part of MSC's caseload.
  • Notifies the DDSO of HCBS termination & inform the terminated individual connecting services.
  • Ensures the consumers fire safety needs are met as stated in the ISP at least annually.
  • Completes with individual and/or his/her advocate a service coordination agreements and this agreement must be reviewed and up-dated at least every six months and documented in the monthly notes.
  • Completes service coordination Activity Plan. (Willowbrook Only)
  • Completes a service coordination observation report once every six months for Willowbrook class members (one must be done with the CAB representative) and as needed for all other individuals in an OPWDD certified ICF, CR or Family Care Home and in any type of certified residence.
  • Participate in admission and discharge planning for the individual as required by OPWDD
  • Attendance standards: flexible hours to include evenings and weekends and emergency situations.
  • Maintains contact with medical, psychiatric, and staff regarding the individuals and advocate’s needs.
  • Awareness of and sensitivity to cultural and socioeconomic characteristics of persons and population served.
  • Maintain 15 hours training per year in accordance with OPWDD relations.
  • Additional responsibilities as deemed necessary by the needs of the individuals being serviced and or the Director of DD Services.

Qualifications

  • Bachelor’s Degree in a health or human services field
  • QMRP experience
  • Two years experience working with people with a developmental disability or
  • One year experience as a service coordinator with any population
  • Bilingual preferred (English/Spanish)
  • Excellent written and oral communication skills
  • Ability to work flexible hours including evening and weekends
  • Has respect and consideration for the individual being served.
