The Pathway of Hope Case Manager provides strengths-based, client-centered case management services to families with children who are seeking to break the cycle of poverty and move toward greater stability and self-sufficiency. This position partners with households over a 6–18-month period to assess needs, develop individualized action plans, connect families to internal and community resources, and support progress toward education, employment, housing, financial stability, and overall family well-being.
