Position Summary:

As a Social Day Case Manager, your role will be to provide overall support to the Social Day/Day Hub and Congregate Meal Site. You will be responsible for intake and ongoing assement of particpating clients,case management, coordinating with program staff to support daily program operations while helping ensure clients receive high quality, person-centered services.

Responsibilities

Responsibilites

  • Conduct participant intake, evaluations, and ongoing assessments, including required COMPASS assessments and documentation.
  • Manage an assigned caseload of seniors while maintaining accurate client files in accordance with funder requirements.
  • Provide referrals, advocacy, service coordination, and support based on participant needs.
  • Communicate client needs and service plan changes to program staff and provide training as needed.
  • Assist with developing monthly activity calendars, schedules, and programs for clients.
  • Maintain accurate attendance, billing, authorizations, and other required program documentation, including PEER Place and NAPIS records.
  • Assist with daily program operations, including greeting/screening participants, serving meals, purchasing supplies, and supporting activities.
  • Assist with volunteers, students, staff meetings, and required trainings while maintaining effective communication with team members.
  • Demonstrate cultural competence, support The Salvation Army’s mission, and perform other duties as assigned

Qualifications

  • Associates Degree preferred 
  • Three to five years of related experience in Human Services
    (e.g., Social Worker, Case Worker, Program Manager, Coordinator, or similar role).
  • Ability to work as a team member and manage team operations and the ability to provide culturally competent practice..
  • Must have valid NYS Driver’s License that meets The Salvation Army's insurance requirements.
