Child/Youth & Family Case Management Services (CPST)
The Salvation Army – Syracuse Area
Casual / As-Needed Hours | $24.00 per hour

The Salvation Army of the Syracuse Area is seeking a compassionate and dedicated Youth & Family Rehabilitation Professional to support children, youth, and families through the CPST Service Program. In this role, professionals provide goal-driven, solution-focused interventions designed to address behavioral health challenges and help children achieve objectives outlined in their individualized treatment plans.

Services are community-based and may include:

  • Case management support
  • Skill-building interventions
  • Family and youth advocacy
  • Functional and behavioral support services
  • Home and community stabilization assistance

 

 

Responsibilities

Key Responsibilities Include:

 
  • Provide community-based rehabilitative and restorative services to a caseload of 10–12 children and families who may benefit more from home and community-based support than traditional office settings. Maintaining caseload minimums and maximums is a critical requirement of this position.

  • Build supportive relationships with children, youth, and caregivers to encourage the everyday use of treatment strategies and individualized goals.
  • Deliver services in a variety of approved community settings, including homes, schools, workplaces, social environments, and community-based agency locations.

  • Coordinate care and collaborate with treatment providers to address identified rehabilitative needs within a comprehensive treatment plan.

  • Provide intensive case management and planning services designed to help prevent crises and promote long-term stability.

  • Support families in accessing and utilizing community resources to restore stability, maintain daily functioning, increase safety, and reduce the impact of current or future crisis situations.

  • Deliver education, rehabilitation, and goal-focused planning services in alignment with each child’s treatment plan or plan of care.

 

 

Qualifications

Qualifications Include:

  • Bachelor’s degree with two years of applicable experience with at-risk children’s populations including: mental health, foster care, child welfare, and/or juvenile justice.  
  • Must have access to own vehicle and valid license that meets The Salvation Army Insurance Requirements.
