The Emergency Practical Assistance Services Program is growing.  Due to this growth, we are now hiring several Case Manager’s to join this already growing Team. 

All positions are full-time, hourly, and eligible for overtime.  It is a community based program where you will work with your clients here in Onondaga County.  Many households' here in

Onondaga County are in need of emergency services.  Family Case Management is a growing field that is rewarding.  Immediate consideration of all applications.

 

SCOPE AND PURPOSE OF POSITION:

The role of the Case Manager is to assess eligibility for housing related practical assistance services.  They provide individual and family case management, supportive counseling, advocacy, life skills education and referrals to other community and mainstream resources to aid in securing and maintaining safe, affordable housing and a stable living situation for the people we serve in our Community!

Responsibilities

  • Conduct intake, evaluations and assessments of prospective clients.
  • Work collaboratively with clients to develop individualized service plans.
  • Provide brokerage, networking, advocacy and referrals to youth service agencies and programs.
  • Carry out individual client sessions, home visits and agency visits.
  • Offer counseling and contact to family members where appropriate.
  • Maintain statistical accountability and fulfill contractual obligations associated with applicable funding sources.
  • Assist with outreach activities as requested or when appropriate.
  • Act as team member and collaborate with other agency programs.
  • Maintain all written information for complete case records.
  • Demonstrate cultural competence, as well as integrate understanding into daily responsibilities and participate in meeting the objectives of The Salvation Army, Syracuse Area Services Strategic Plan.

 

Qualifications

  • Bachelor’s degree in human services preferred. Equivalent education and experience will be considered.
  • Experience with word processing, data base entry, internet and Microsoft Excel preferred.
  • Professional position includes expectation that a flexible work schedule be maintained according to individual client needs including late afternoon/evening hours as needed.
  • Must have strong time management skills and have the ability to work independently.
  • Must have a NYS Drivers License that meets The Salvation Army insurance requirements.
