Overview

Location: New Britain Men’s Shelter (New Britain, CT)

Status: Non-exempt, Full-time

Hours Per Week: 37.5

Schedule: Monday – Friday, 8:00 am – 3:30 pm

 

Scope of Position: Assist homeless youth, ages 16 – 24, in identifying and removing barriers to securing safe housing. Collaborate with the Central Coordinated Access Network (CCAN) and partner agencies to deliver outreach, assessment, referrals, and diversion services, expanding and complementing those already offered by the CAN. 

Responsibilities

  • (Re)connect youth with family members, if deemed appropriate by the client.
  • Assist clients with paperwork and documentation, including benefits and additional housing interventions.
  • Conduct age-appropriate intake appointments.
  • Conduct CAN appointments in the community and all towns of the Central CAN to accommodate the needs and preferences of young people in crisis, including after regular business hours.
  • Partner with public libraries in Central CAN to offer appointments at locations based on the preferences of youth.
  • Develop individual housing plans with youth by building rapport and trust.
  • Empower and encourage clients to be change agents in their own lives by focusing on and building up their resiliency, strengths, and protective factors.
  • Guide clients through the housing response system, including providing options for securing permanent housing.
  • Assist clients with referrals and linkages to various services, including employment and educational programs, behavioral healthcare, primary care, basic needs, financial literacy, and life skills.
  • Maintain a minimum caseload of 10 homeless youth seeking shelter or housing.
  • Maintain client files, statistics, and HMIS database in accordance with agency and contract standards.
  • Pair off clients for shared living arrangements when feasible and only when there is client interest.
  • Practice persistent engagement to achieve a minimum of 50% show rate at CAN appointments and to keep youth engaged in services to eliminate or compensate for housing barriers during the approximately 90 days they are in case management.
  • Regularly participate in youth-focused community meetings.
  • Schedule at least six CAN appointments per week with homeless or unstably housed youth through 211.

Qualifications

  • High school diploma or GED required; a bachelor’s degree is preferred.
  • Availability to work nontraditional hours to meet client needs required.
  • Bilingual in Spanish/English preferred.
  • Familiarity and experience with service models such as Positive Youth Development, Motivational Interviewing, Housing First, Harm Reduction, and Trauma-Informed Care are preferred.
  • Lived experience with homelessness preferred.
  • Minimum of at least one year of experience in social services required.
  • Must complete KeepSAfe and other training requirements upon hire.
  • Valid driver’s license or access to transportation required.
