Overview

To assess and meet with each resident regularly, set individual plans for independent living, and assist them in

obtaining needed services in the community for the purpose of securing stable permanent housing.

Responsibilities

  1. Responsible for implementing all Carey House policies and procedures related to maintaining an orderly
  2. and safe living environment for all residents
  3. Complete an assessment with each client, identify needs, and develop service and action plan with client
  4. Provide supportive guidance as it relates to the agreed upon service and action plan
  5. Demonstrate empathy and a positive attitude in all interactions with clients, visitors, and volunteers
  6. Maintain all records and documents relative to the interview/guidance meetings and subsequent progress of clients
  7. Meet with Director and other paid staff regularly with updates pertaining to each client’s progress and needs
  8. Inform Director and other paid staff of any escalating situations.
  9. Maintain confidentiality of all information regarding residents or any information discussed in confidence (with            the exception of The Salvation Army staff)
  10. Responsible for the intake process of new residents
  11. Responsible for discharge of residents (with authorization) when necessary
  12. Read shift logs and attend briefings and attend staff meetings as scheduled
  13. Keep accurate and updated records
  14. Administer drug testing per shelter policies
  15. Provide continuous coverage by remaining on duty until properly relieved
  16. Update and maintain NH HMIS data records
  17. Weekend shifts as assigned
  18. Other duties as assigned
  19. Responsible for being on time and fulfilling all of your scheduled shifts.
  20. Meet with outside agencies when needed to assist the client's needs.
  21. Enforce and abide by all Salvation Army policies and procedures.
