Overview:  The Social Day Program for Seniors here at The Salvation Army is currently hiring for the position of Case Manager!  This is a full-time opportunity and a wonderful program where you can help our Seniors. The primary purpose of this position is to provide overall support of the Social Day/Day Hab and Congregate Meal site. This position is responsible for intake and ongoing assessment of social day clients, case management, working with program support staff with the completion of various tasks to support the delivery of quality services to clients.

Responsibilities

Operational Functions

  1. Assist with greeting and screening all employees, clients, and visitors to the program, conducts appropriate health screening procedures prior to admittance to the program areas.
  2. Conduct comprehensive intake, evaluations, and assessments of prospective participants including intake paperwork.
  3. Assists with answering inquiries about programs provided by senior services either via phone or outreach activities.
  4. Responsible for an assigned caseload of seniors: maintain client files per funder regulations and conduct ongoing semi-annual COMPASS assessments.
  5. Provide referrals, brokerage, networking, and advocacy.
  6. Provide ongoing training to program support staff on client needs and service plan changes.
  7. Assist with the development of the center schedule and activities.
  8. Attend staff meetings and required training sessions.
  9. Assist with the collection of daily billing documentation such as authorizations from funders, daily attendance to be submitted to finance and funders.
  10. Purchasing of supplies needed for any program activities monthly, including snack items.
  11. Demonstrate cultural competence, as well as integrate understanding into daily responsibilities and participate in meeting the objectives of The Salvation Army, Syracuse Area Services Strategic Plan.
  12. Assist with program support duties/activities as needed or assigned in the program.
Planning/Organizing Activities
  1. Offers suggestions and comments on program performance/efficiency.
  2. Work with Program staff to create the monthly activity calendar and distribute it to all clients.

 Documentation Functions

  1. Maintains all client files in their caseload according to funder requirements.
  2. Completes all intake paperwork and ongoing assessments per funder requirement.
  3. Assists with logging in participants and ensuring the accuracy of attendance billing data.
  4. Uploads required documentation into the county system PEER Place.
  5. Completes NAPIS registration forms for NYSOFA Congregate clients.

Other

  1. Assist to direct volunteers and students during activities.
  2. Maintain good communication skills and rapport with team members and various departments and informing supervisor of any problems or concerns.
  3. Assist with serving meals when needed.

Qualifications

 

  • Min. of a High School Diploma/GET with 1 year of related experience working with Seniors
  • Preferred, 2 year degree with 2-4 years of related experience working with Seniors
  • Minimum of one-year experience working with Seniors and/or Social Day Programs is desired.
  • A strong ability to shift focus frequently and re-prioritize due to interruptions.
  • Valid NYS Driver License that meet’s The Salvation Army’s insurance requirements.
