General Purpose Of Job:
The POH Initiative is a directive from The National Salvation Army command and will be implemented at every corps unit by 2019. The POH Initiative entails case management over a period of 6-18 months of households with children who have a desire to change their trajectory of being in poverty.
The focus of case management is to enable the families to resolve barriers to and work towards self-sufficiency which mainly includes their education and employment needs.
With the guidance, and support of the divisional POH regional coordinator, the case manager will use the following approaches when working with clients: Strengths-based Model, Stages of Change Model, and motivational interviewing skills.
At the Norwalk Ohio Corps, the case manager will have a caseload of up to 4-6 families at a time, and will meet
with the families for 1–2-hour sessions weekly or bi-weekly. Documentation is entered in the ServicePoint data system in a timely manner, statistics are tracked for monthly reports, and client assistance expenses are accurately recorded per grant requirements.
Primary Essential Responsibilities: (general outline provided, not complete list of duties)
Provide services to the client family via the case management steps.
Maintain a case record accurately and timely in Service Point database system.
Track statistics and contingency fund use.
Collaborate with divisional POH regional coordinator and fellow POH case managers.
Education / Experience / Qualifications:
Bachelors or Masters of Social Work or other human service related degree. Case Management experience is a plus. Licensed Social Worker preferred
