Overview

Position: NDNH Case Manager

Department: Philadelphia Social Service Ministries; New Day New Home; Work Location: Philadelphia County                  

Availability of Position: Immediate

Schedule: Full-time (40 hours a week), Monday- Friday 9 am – 5 pm

Reporting Relationship: New Home House Manager

Salary and Grade: 76/ Non-Exempt

Position Overview: The New Day New Home Case Manager will provide client-centered, trauma-informed, clinical case management to survivors of human trafficking residing with the New Day New Home program. The New Home Case Manager is to develop individualized, trauma-informed and culturally sensitive service plans with victims of HT residing within the home and support clients through clinical case management services throughout the duration of each resident’s stay. The New Home Case manager is to apply a client-centered approach to advocating for client access to resources as needed based on service plans for the development of life skills and the meeting of needs such as employment, education, training, detoxification, mental health, physical health, legal, family, and financial. The New Home Case Manager is to work closely with the House Manager to cultivate a safe and trauma-informed living environment that promotes the holistic healing, growth, and development of residents. 

Responsibilities

Duties:

  • Abide by The Salvation Army Social Service Standards & the NASW Code of Ethics.
  • Perform related duties as assigned.
  • Conduct initial screening, intake, and assessment of newly identified residents of the home. 
  • Ensure timely coordination of referrals to resources and the meeting of immediate needs upon each client’s arrival to the home.
  • Develop individualized, client-centered, trauma-informed, and culturally sensitive service plans for self-sufficiency with victims of human trafficking living at program 
  • Coordinate short and long term aftercare services for residents exiting NDNH and going into rapid rehousing programming. 
  • Support clients in reintegration into the community to develop a sense of voice and regain power and control after suffering forms of severe trauma. 
  • Manage a direct service caseload of 8-16 clients at any given point in time
  • Participate in managing a 24/7 hotline for response to victims of HT and coordinate services needed for all potential victims.
  • Participate in the facilitation of programming and groups within the New Home program, to include SELF, spirituality, expressive art, and social enterprise groups.  
  • Keep up to date case notes and ensure client confidentiality in all forms of communication.
  • Submit timely reports on case management duties into the Service Point database, and other internal and external reporting as is necessary 
  • Participate and conduct trainings for awareness events as needed for internal or external community based partners in Anti-Human Trafficking work.
  • Establish and maintain connections with community resources 
  • Engage clients in life skills development activities to include meal planning, grocery shopping, budgeting, time management, and others based on individualized service plan as a part of ongoing case management

Qualifications

Requirements:

As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check. MSW degree or BSW with minimum two years experience working in human trafficking or a related field (e.g. domestic violence, immigrant rights, workers’ rights) in a role that involves service provision, training, and/or advocacy; Experience with trauma informed care preferably a working knowledge of Sanctuary Model for Trauma informed services.  Must have an understanding of the issue of Domestic and Foreign Human Trafficking. Must have a valid driver’s license and reliable transportation to travel from site to site as may be needed in the performance of duties. Professional-level communications skills verbal and written. Bilingual in Spanish preferred. Must possess an understanding of and support the mission of The Salvation Army.
