The role of a Case Manager for the Street Outreach Program here at The Salvation Army provides individual case management/social work services to runaway and homeless youth and young adults, assisting with crisis intervention, housing stabilization, education, employment, mental health referrals and other related life issues.

Responsibilities

Essential Duties:

  • Provide comprehensive intake, assessment and evaluation of prospective clients.
  • Devise and implement individualized service and support plans and intervention strategies utilizing client input and maximizing client choice.
  • Initiate and maintain regular contact and street outreach with assigned clients, conduct home visits of significant duration to assist them with acquiring skills to meet their goals of self-sufficiency.
  • Provide therapeutic support and crisis intervention.
  • Street Outreach activities will take in the general community as well as in specifically targeted areas and settings, such as street corners, parks, community centers, sporting events, home visit locations.
  • Maintain statistical accountability and accurate written information for complete case records and provide ongoing assessments and case recommendation. Compile data and complete reports.
  • Collaborate with other services providers, family and other client-identified individuals to arrange for delivery of services and referrals.

 

 

Qualifications

Qualifications:

  • Bachelor’s degree in human services or related field is preferred.
  • High School Diploma or Associate’s degree in conjunction with increased years of relevant experience will be considered 
  • Specific experience in mental health services and knowledge of community resources required.
  • Professional position includes expectation that a flexible work schedule will be maintained according to individual client needs including late afternoon/evening hours as needed.
  • Valid NYS Driver License that meets The Salvation Army's insurance requirements.
