Overview

The Case Manager at the Stapleton Corps will work to develop partnerships with other social service agencies, churches and other not-for-profits across Staten Island while strategically utilizing those relationships to cultivate comprehensive, individualistic plans of recovery and healing for the families being served through the Social Ministries of The Stapleton Corps.

Responsibilities

  • Cultivate partnerships with other social service providers, churches, and other not-for-profits across Staten Island that offer services that could assist clients being
  • Performs casework duties, which could include but is not limited to meeting with clients seeking agency services, conducting intake and assessments, brief counseling, and information and referral
  • Develops comprehensive service plans and makes periodic reports to supervisor on the progress of each
  • Carries a caseload and performs record keeping functions such as detailed progress notes, and census statistical
  • Maintains all records with strict
  • Refers residents to agencies and programs as necessary and establishes a rapport and professional relationship with these
  • Attends staff meetings as requested by the Corps
  • Attend training sessions and workshops around the NYC area as requested by Corps
  • Create a process for measuring the impact of personalized plans created for clients and measure each case at least two times per year. (not sure what this means)
  • Prepares workshops and other educational opportunities for (Maybe add some specificity to this- what kinds of things would you want this person to do? Workshops and education are vague)
  • Works closely with other staff at the Stapleton
  • Review and determine approval for various forms of support (clothing vouchers, rent assistance etc.) based upon budget
  • Oversee the Pantry Aide. Including: scheduling, payroll input, disciplinary actions, etc)
  • Consistently reports to work on time and is ready to begin work at the start of the shift
  • Always provides proper notification when late
  • Demonstrates reliability when on-call. Available to work overtime when necessary (only non-exempt employees).
  • Demonstrates the ability to recognize priorities in organization of workflow.
  • Able to perform duties independently, with minimal need of direct
  • Maintains the confidentiality of clients, shelter/program, agency and/or The Salvation
  • Reports to work well-groomed and in compliance with dress
  • Demonstrates the ability to interact in a positive and helpful manner with clients, visitors, other staff, volunteers, and co-workers.
  • Reflects commitment to building a supportive work environment and maintains a positive attitude at the workplace and towards
  • Projects a good image in dealing with the public and clients.
  • Is willing to make an extra effort to help build a quality case management

Qualifications

  • Bachelor Degree in Human Services or a related
  • Demonstrated experience and knowledge working with underserved populations on Staten Island or the surrounding boroughs, which include children and
  • MSW candidate preferred, but not required
