Come join our team as a Case Manager! 

The Safer Streets program at The Salvation Army is seeking two full time case managers. Case managers will act as mentors, role models and life coaches for youth (ages 18-25) experiencing difficulties and hardships in their daily lives.  Case Managers may work non-traditional hours, including some evenings and weekends.

Responsibilities

Essential Duties:

  • Provide comprehensive intake, assessment and evaluation of prospective clients.
  • Devise and implement individualized service and support plans and intervention strategies utilizing client input and maximizing client choice.
  • Initiate and maintain regular contact with assigned clients, and conduct home visits.
  • Provide therapeutic support and crisis intervention.
  • Maintain statistical accountability and accurate written information for complete case records and provide ongoing assessments and case recommendation. 
  • Collaborate with other services providers, family and other client-identified individuals to arrange for delivery of services and referrals.
  • Participate actively in case conferences, staff meetings, supervision, professional/in-service training sessions and collaborative meetings as scheduled.
  • Maintain connections with outside agencies and ensure professional representation of the agency in the community.
  • Assist with client interaction during street outreach and drop-in center scheduled activities.
  • Assist with light cleaning and housekeeping duties throughout program areas in order to maintain professional environment for clients.

 

 

 

Qualifications

Qualifications:

  • High School Diploma and experience within field or Associate’s degree in conjunction with increased years of relevant experience will be considered 
  • Specific experience in mental health services and knowledge of community resources is preferred.
  • Professional position includes expectation that a flexible work schedule will be maintained according to individual client needs including late afternoon/evening hours as needed.
  • Valid NYS Driver License that meets The Salvation Army's insurance requirements.
