SCOPE AND PURPOSE OF POSITION:

Oversee the activities of agency Care Managers to assure the provision of high-quality, family- driven and youth-guided service that is in accord with the policies and regulations of the agency, the Health Home, and the NYS Department of Health.  Support Care Managers in their efforts to help families and children advance the health care status of youth eligible for Health Home service.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Observe agency personnel policies and assure compliance by supervisees.
  • Oversee comprehensive intake, evaluation and clinical assessments of prospective clients.
  • Actively initiate referrals for Care Management Program to ensure Care Managers caseloads are at capacity.
  • Review documentation by supervised care managers, confirming regulatory adequacy and timeliness.
  • Assure that care management activity is consistent with the principles, regulations, and policies associated with Health Home care management.
  • Provide consultation to care managers concerning outreach, enrollment, the development of Plans of Care, and the conduct of continuing care management activity.
  • Assure care managers complete and maintain necessary training.
  • Participate in mandatory and optional training.
  • Evaluate impact of care managers’ activity on the health care status of clients, using QI data and observation; assist care managers as appropriate in improvement of performance.
  • Supervise student interns and mentor volunteers as assigned.
  • Fulfill 24-hour on-call responsibilities as assigned and all other duties as assigned by the Director.
  • Meet with Corps Officer monthly to discuss pertinent information.

Qualifications

SPECIAL SKILLS, CERTIFICATES, LICENSES, REGISTRATIONS.

 

•    MSW/MA and in clinical field and three - five years of experience working with youth and families preferred, as well as knowledge of family systems

•    Clinical license preferred and two or more years of supervisory experience preferred 

•    Fluent with the concepts of family-driven, youth-guided care

•    The concept of family as natural care manager

•    Understands that improvement in the health care status of the youth is the single most important metric

•    Can maintain a detailed awareness about the development and execution of multiple Plans of Care

•    Able to communicate effectively with a diverse caseload of supervisees; solid written and verbal skills

•    Comfortable with the analysis and utilization of Quality Improvement data

•    Understands care manager job descriptions and variation according to acuity and service specialty (e.g., EI) as appropriate

•    Must have valid license that meets The Salvation Army insurance requirements

•    Must have access to own vehicle for therapeutic outreach services

•    Provide physical assistance/or intervention with children/youth

•    Perform all duties associated with job responsibilities
