Position Details:  As the Shelter Case Manager for our Women's Shelter you will have a career opportunity where you will be able to assist our clients who are homeless/in crisis by using short-term case management, support counseling, advocacy and by referring to community resources. Our Case Manager's provide individual and family case management to our clients.  They assist cleints with life skills education, and mainstream resources to aid in securing and maintaining safe, affordable housing along with a stable living situation.

 

Benefit Overview:   Health Benefit Program.  This is made available to our employees after they complete their first ninety (90) days of employment.   13 paid holiday's, Personal Time, Vacation and Sick Paid Time Off (PTO).  This position would also qualify as part of the Loan Forgiveness Program.  We also offer some unique benefits=On Site Daycare, Marriage Benefit, Orthodontia on the Dental Plan, Hearing Aides and free "Will" Services.   

 

Who we are:   The Salvation Army’s Women’s Shelter provides temporary emergency housing for adult women without children, who have serious mental health problems and psychiatric disabilities. Crisis counseling, comprehensive social work support services, mental health services and linkages to community resources are provided to help the women obtain housing stability, income supports, mental health treatment and supportive services. The Women's Shelter has 15 beds and 16 subsidized one bedroom apartments.

 

Responsibilities

Overview of job responsibilities:

  • Conduct comprehensive client assessments and work with the client/family to develop an individualized independent living plan. Identify needs and make referrals to community providers.
  • Supervision of interns including completion of evaluations, verification of hours, and orientation to the program.
  • Provide empathetic non-judgmental counseling and support in crisis situations and as a part of short-term case management. Monitor/manage agreements with clients. Contract with clients to provide reasonable accommodations for those unable to abide by routine agreements.
  • Actively monitor client progress for their Independent Living Plan goals.
  • Direct weekly client case reviews and provide case consultation to other staff members related to client needs.
  • Maintain service statistics for internal and external reporting, using database tools. Maintain case records to meet standards (client rights/ confidentiality, assessment-based service planning, clear and concise individual case narratives, documentation of referrals, discharge summaries).
  • Network with community providers and provide advocacy on behalf of client. Represent program and agency in the community at meetings and case conferences.
  • Provide limited coverage for the Assistant Program Manager and/or PH CM when he/she is not available. 
  • Answer telephone and respond to inquiries. Share on-call support for 24-hour operations.
  • Demonstrate cultural competence, as well as integrate understanding into daily responsibilities and participate in meeting the objectives of The Salvation Army, Syracuse Area Services Strategic Plan.

Qualifications

Qualifications:

  • Associate degree in human services with one year of experience required.
  • BA/BS and 2 years of experience preferred.  
  • Experience/knowledge of mental health, substance abuse issues and community resources.
  • Strong engagement and assessment skills and ability to plan and provide appropriate interventions.
  • Team oriented and service oriented in approach.
  • Excellent communications skills both verbal and written.
  • Proficient in computer use to include Microsoft Office and data entry.
  • Must have valid NYS driver’s license.

Please note:  This position could require the individual to be placed within the rotating schedule of "On-Call".  Flexibility in ones work schedule to meet the demands of the Women's Shelter is required.  Evenings, Holiday's and/or Weekends could potentially be a part of the Shelter Case Manager's schedule.
