Case Manager, Youth Shelter

Barnabas House, The Salvation Army of the Syracuse Area

Full-time, $21.00/hour with access to our full time Health & Wellness Benefit Program

 

Overview:  Barnabas House/Shelter is a co-ed group facility offering short-term emergency housing to young adults, ages 18-24.  They can receive shelter services for up to 2-4 weeks, which often times allows them to become enrolled in our long-term transitional housing apartments or our rapid rehousing rental assistance program. We are currently hiring for the position of Case Manager.  A Shelter Case Manager provides individual case management/social work services to the Youth and Young Adults living at Barnabas.  They assist with crisis intervention, housing stabilization, education, employment, mental health referrals and life related issues.

Responsibilities

  • Provide comprehensive intake, assessment and evaluation of prospective clients. Devise and implement individualized service and support plans and intervention strategies utilizing client input and maximizing client choice.
  • Initiate and maintain regular contact with assigned clients, conduct home visits of significant duration to assist them with acquiring skills to meet their goals of self-sufficiency. 
  •  Provide therapeutic support and crisis intervention.
  • Maintain statistical accountability and accurate written information for complete case records and provide ongoing assessments and case recommendation. Compile data and complete reports.
  • Collaborate with other services providers, family and other client-identified individuals to arrange for delivery of services and referrals.
  • Participate actively in case conferences, staff meetings, supervision, professional/in-service training sessions and collaborative meetings as scheduled. Maintain connections with outside agencies and ensure professional representation of the agency in the community.
  • Assist with client interaction during street outreach and drop-in center scheduled activities.
  • Assist with light cleaning and housekeeping duties throughout program areas in order to maintain professional environment for clients.
  • Demonstrate cultural competence, as well as integrate understanding into daily responsibilities and participate in meeting the objectives of The Salvation Army, Syracuse Area Services Strategic Plan.

Qualifications

  • Bachelor’s degree in human services with at least 3 years of experience working with young adults.
  • Experience and knowledge of community resources.
  • Must have valid New York State driver’s license that meets The Salvation Army insurance requirements.
  • Flexible scheduling to accommodate occasional evening, overnight or weekend hours.
  • Ability to successfully complete the background screening, physical/TB Test & Drug Screen
